Whether your employer offers your direct-payment vision benefit plan for exam only or for exam and optical needs, you will gain additional discounts by using one of our contracted network professional providers.
Exam Only Benefit Plan:
Exam: Paid in full with $10 copayment paid at time of visit.
Optical discounts:
• 10% discount on first pairs of eyeglasses.
• 20% discount on additional pairs of eyeglasses in the same calendar year from the same provider.
Exam and Optical Needs Benefit Plan:
Exam: Paid in full with $10 copayment paid at time of visit.
Optical Discounts in addition to optical benefit amount offered by your employer:
• 15% discount on total frame amount
• 10% discount on premium eyeglass lens upgrades
• 20% discount on additional pairs of eyeglasses in the same calendar year from the same provider.
Although you are responsible for contact lens fitting fees, your optical needs benefit amount may be applied to those evaluations.
Remember: You are not limited to using your optical needs benefit to either spectacle or contact lenses. If you do not need a new frame one year, you may use your entire optical needs benefit toward lenses only, or split your benefit between spectacle lenses and contact lenses. You may utilize your benefit for your specific needs.
Please note: Optical needs benefit amounts must be billed in a single claim filing. However the exam may be billed separately from the materials benefit. No unused exam or materials benefit will carryover from one calendar year to the next.
