Benefit Plan Options for Employers
For Direct-Payment to Providers

PLAN # 1: Exam only

Exam benefit:
Employer pays up to $ 50 (not to exceed the total exam fee)
Employee pays $ 10 copayment

Materials benefit:
• 10% discount on first pair of eyeglasses
• 20 % discount on unlimited additional pairs of eyeglasses in the same
calendar year from the same provider

PLAN # 2: Exam and Optical Needs

Exam benefit:
Employer pays up to $ 50 (not to exceed the total exam fee)
Employee pays $ 10 copayment

Materials benefit choices:

A. $ 75 benefit from employer
B. $100 benefit from employer
C. $150 benefit from employer
D. $200 benefit from employer

Network providers also apply:
• 15 % discount on total frame amount
• 10 % discount on premium eyeglass lens upgrades
• Employee is responsible for contact lens fitting fee, however benefit
amount may apply to those evaluations
• 20 % discount on unlimited additional pairs of eyeglasses in the same
calendar year from the same provider

All plans follow calendar year benefits extending January 1st to December 31st.

Materials benefit amounts must be billed in a single claim, however exam may be billed separately from the materials benefit. No unused materials benefit will carryover after processing in the same calendar year or the following year’s benefit period.

Professional Eye Care Administrators, Inc. will process claims from employee’s choice of participating professional provider on the network list.

A minimal set-up fee of $1 per covered person if provided in electronic format (minimum of $25) will apply to employers when plan is initiated or when additions or deletions occur. (Set-up fee of $2 per covered person will apply if information is provided via paper.)

No additional claim processing or administrative fees will come from Professional Eye Care Administrators, Inc. to employers outside of the indicated set-up fees.
